Following is information about how to file a request to view records of the Kentucky Division of Water.

To obtain access to Division of Water files and information, contact Jerry Milburn in the KPDES file room by phone at (502) 564-3410, by fax at (502) 564-5105 or by e-mail at jerry.milburn@ky.gov.

If you need to view files in the custody of the Division of Water, contact Jerry Milburn as above so he can gather files and set an appointment for you to view them. If you should need to make copies of any documents, there is a copier in the file room which you may use for 10 cents per page.

To review Division of Water files and information, you need to complete a Freedom of Information form and fax it or mail it to Kentucky Division of Water, 14 Reilly Road, Frankfort, KY 40601, Attention: Jerry Milburn.
